Nurse Practitioner Community Health (Scarborough, ON)
Full-Time, Permanent | 35 hours/week

About the Opportunity
We are currently seeking a Primary Health Care Nurse Practitioner to join a community-based primary care team in Scarborough, ON. This role focuses on delivering accessible, equitable, and culturally responsive care to children and families, with an emphasis on health promotion, illness prevention, and chronic disease management.

Key Responsibilities

• Provide comprehensive, family-centred primary health care within your full NP scope of practice.
• Assess, diagnose, and manage acute and chronic conditions for clients across the lifespan.
• Conduct client interviews, physical exams, and develop evidence-based treatment plans.
• Administer immunizations according to Ontarios publicly funded immunization schedule.
• Maintain accurate and confidential client health records in compliance with CNO standards.
• Collaborate with physicians and allied health professionals as part of an interdisciplinary team.
• Participate in case conferences, peer reviews, and health promotion/outreach initiatives.
• Support continuous quality improvement and best practice activities.
• Provide home visits and extended hours (evenings/weekends) as needed to meet client needs.

Qualifications

• Masters Degree in Nursing with Primary Health Care Nurse Practitioner (NP-PHC) certification.
• Current registration with the College of Nurses of Ontario (Extended Class) in good standing.
• Minimum 35 years experience as a Nurse Practitioner in primary care, community health, or a similar setting (CHC, FHT, NP-led clinic).
• Current CPR (HCP) certification.
• Strong assessment, clinical reasoning, and documentation skills.
• Excellent interpersonal and communication abilities; able to work collaboratively within a team.
• Experience serving culturally, socially, and economically diverse populations.
• Familiarity with EMR systems (Nightingale on Demand considered an asset).
• Valid Ontario drivers license and access to a vehicle considered an asset.

Compensation
Competitive hourly rate of $51.29 $72.65, commensurate with experience, plus benefits.

Schedule
Full-time, 35 hours per week, with some evening and weekend shifts as required.
