About the Role

We are seeking a full-time Occupational Therapist (OT) Case Manager to join an interdisciplinary ABI (Acquired Brain Injury) Outreach Team. This team provides community-based support to individuals living with ABI and their families across home and community settings.

Working collaboratively with a multidisciplinary team, you will play a key role in supporting clients with complex healthcare needs through assessment, care planning, and ongoing case management.

Key Responsibilities

Client Care & Clinical Practice

  • Conduct assessments and provide care in homes, hospitals, hostels, and community settings
  • Develop and implement individualized care and treatment plans based on client and family goals
  • Apply therapeutic approaches grounded in health promotion and disease prevention
  • Create and evaluate client education materials
  • Work both independently and collaboratively within an interdisciplinary team

Community & Partner Engagement

  • Build strong relationships with healthcare providers and community partners
  • Advocate for client needs across the healthcare system
  • Provide consultation and guidance to internal teams and external partners
  • Participate in mentorship and clinical supervision opportunities

Operations & Administration

  • Utilize electronic health records (EHR) and shared care plans effectively
  • Meet performance indicators and contribute to data collection and reporting
  • ADP authorization experience considered an asset

Qualifications

Education & Experience

  • Current (or eligible for) registration with the College of Occupational Therapists of Ontario
  • Minimum 2 years of experience in community-based occupational therapy preferred
  • Experience working with Acquired Brain Injury and/or mental health populations is an asset

Skills & Competencies

  • Strong communication, collaboration, and relationship-building skills
  • Excellent critical thinking and problem-solving abilities
  • Ability to work independently and manage time effectively
  • Proficiency with computers and electronic health record systems
  • Knowledge of the healthcare system related to ABI is preferred
  • Additional language skills are considered an asset
  • Commitment to continuous learning and professional development

Working Conditions

  • Community-based role with travel required (access to a vehicle is an asset)
  • Work conducted in client homes, hospitals, hostels, and other community settings
  • Standard hours with flexibility for occasional evenings/weekends

Compensation

  • $41.98 – $48.98 per hour
