Registered Nurse
Job Description:
- Conduct initial assessments of patient's health status and develop individualized care plans based on their needs and preferences.
- Administer medications, wound care, and other medical treatments as prescribed by physicians.
- Educate patients and their families on disease management, medication administration, symptom recognition, and preventive care measures.
- Collaborate with physicians, therapists, social workers, and other healthcare professionals to coordinate patient care and ensure seamless transitions between healthcare settings.
- Document all patient interactions, assessments, interventions, and outcomes accurately and timely in electronic medical records (EMR) or other designated systems.
- Respond promptly to changes in patients' conditions, medication issues, or other emergencies, and communicate effectively with the healthcare team and family members.
- Advocate for patients' needs and rights, ensuring they receive appropriate and timely healthcare services and support.
- Maintain up-to-date knowledge of best practices, guidelines, and regulations related to home healthcare and nursing practice.
- Participate in quality improvement initiatives, clinical meetings, and continuing education activities to enhance professional growth and competency.
Qualifications:
- Valid Registered Nurse (RN) license in the state of practice (DOH license)
- Bachelor of Science in Nursing (BSN) preferred.
- Minimum of two years of clinical nursing experience, preferably in Medical/Surgical ward, Pediatric ward, General ward ICU, PICU, ER, HDU.
- Valid BLS Certification.
- Strong assessment, critical thinking, and problem-solving skills.
- Excellent communication and interpersonal abilities.
- Ability to work independently and as part of a multidisciplinary team.
- Compassionate and empathetic attitude towards patients and their families.
