• Conduct initial assessments of patient's health status and develop individualized care plans based on their needs and preferences.
• Administer medications, wound care, and other medical treatments as prescribed by physicians.
• Educate patients and their families on disease management, medication administration, symptom recognition, and preventive care measures.
• Collaborate with physicians, therapists, social workers, and other healthcare professionals to coordinate patient care and ensure seamless transitions between healthcare settings.
• Document all patient interactions, assessments, interventions, and outcomes accurately and timely in electronic medical records (EMR) or other designated systems.
• Respond promptly to changes in patients' conditions, medication issues, or other emergencies, and communicate effectively with the healthcare team and family members.
• Advocate for patients' needs and rights, ensuring they receive appropriate and timely healthcare services and support. 
• Maintain up-to-date knowledge of best practices, guidelines, and regulations related to home healthcare and nursing practice. 
• Participate in quality improvement initiatives, clinical meetings, and continuing education activities to enhance professional growth and competency. 

Qualifications:

• Valid Registered Nurse (RN) license in the state of practice (DOH license)
• Bachelor of Science in Nursing (BSN) preferred.
• Minimum of two years of clinical nursing experience, preferably in Medical/Surgical ward, Pediatric ward, General ward ICU, PICU, ER, HDU.
• Valid BLS Certification.
• Strong assessment, critical thinking, and problem-solving skills.
• Excellent communication and interpersonal abilities.
• Ability to work independently and as part of a multidisciplinary team.
• Compassionate and empathetic attitude towards patients and their families.
