Location: Portland, OR - Hybrid. This is a field-based home health position primarily serving patients in Gresham and surrounding communities.

Note: Candidates must have an active Oregon Physical Therapist (PT) license. Home health experience, 1+ year of Physical Therapist experience, a Master's or Doctorate degree in Physical Therapy, BLS/CPR certification, a valid driver's license and reliable transportation, HCHB/Homecare Homebase experience, and OASIS experience are preferred.

Job Description

  • Evaluate patients in their homes and assess mobility, strength, balance, coordination, pain, and functional limitations.
  • Develop individualized physical therapy treatment plans based on each patient's needs and goals.
  • Provide therapeutic exercises and techniques designed to improve mobility, strength, function, and independence.
  • Educate patients and caregivers on exercises, safety precautions, and self-care strategies.
  • Monitor patient progress and adjust treatment plans as appropriate.
  • Collaborate with nurses, physicians, therapists, caregivers, and other members of the interdisciplinary team.
  • Complete accurate and timely clinical documentation.
  • Travel throughout Gresham and surrounding communities to provide high-quality in-home care.

Qualifications

  • Active Oregon Physical Therapist (PT) license required.
  • Home health Physical Therapist experience preferred.
  • 1+ year of Physical Therapist experience preferred.
  • Master's or Doctorate degree in Physical Therapy preferred.
  • Current BLS or hands-on CPR certification preferred.
  • Valid driver's license and reliable insured vehicle preferred.
  • HCHB/Homecare Homebase experience preferred.
  • OASIS experience preferred.
