Job Scope:

We are looking to recruit experienced Registered Nurse to join the team at a hospital in Bristol. In this role, you will be supporting elderly and dementia patients by ensuring that all necessary paperwork and processes are completed to facilitate safe and timely discharges back to their homes or appropriate care settings.

As a Registered Nurse, you will act as a key link between the hospital, care providers, and patients families. You will provide expert advice on care packages and discharge pathways, ensuring a smooth transition for patients from hospital to community care. Additionally, you will be responsible for maintaining effective communication with the wider multidisciplinary team, representing care providers while remaining independent in your assessments.

Your role as a Registered Nurse will include conducting professional and timely patient assessments, identifying barriers to discharge, and working collaboratively to overcome them. You will also ensure compliance with all relevant discharge processes and documentation, such as the Transfer of Care Form, to facilitate smooth transitions from hospital to care settings.

Do you have?

  • Registered Nurse (RGN) with experience in a nursing home or hospital discharge settings.
  • Strong communication and liaison skills with multidisciplinary teams.
  • Expertise in navigating discharge processes and care pathways.
  • Ability to work independently as a neutral Discharge Assessor, ensuring patients receive the best possible care.
  • Diploma in Management - Level 5 CM, or an equivalent leadership qualification, desirable.
  • In-depth knowledge of the complexities of caring for older people, or a university-accredited qualification related to frailty.
