Base salary: generally lands around $280,000–$300,000
Bonus structure: up to 25% total
- Up to 2.5% quarterly tied to territory performance
- Up to 10% annual tied to territory performance
- Up to 5% company annual bonus
Job Overview:
We are seeking a Market Physician Executive (MPE) to lead clinical operations and population health initiatives within an assigned market. This physician leader will oversee a multidisciplinary care model focused on delivering in-home care to patients with complex chronic conditions, particularly those living with chronic kidney disease and related metabolic disorders.
The MPE is responsible for guiding clinical strategy, overseeing care delivery, and driving improved patient outcomes through evidence-based clinical pathways. Working closely with advanced practice providers, nurses, social workers, pharmacists, and community partners, the MPE ensures high-quality care while improving patient experience, provider satisfaction, population health outcomes, and overall cost efficiency.
Key Responsibilities:
- Lead and oversee the in-market multidisciplinary care team including APPs, RNs, social workers, and pharmacists
- Implement and ensure adherence to evidence-based clinical pathways and the organization's model of care
- Review, approve, and co-sign advanced practice provider encounters and patient care plans
- Provide clinical leadership for population health initiatives aimed at improving outcomes and reducing healthcare utilization
- Deliver direct and indirect patient care including diagnosis, treatment, and care coordination
- Collaborate with community physicians, facilities, and healthcare partners in patient care decisions
- Conduct peer-to-peer clinical discussions with health plans, providers, and healthcare facilities
- Order labs, referrals, and care interventions to improve patient outcomes and close care gaps
- Lead daily clinical review rounds for high-risk patients and concurrent care management
- Oversee operational performance including patient engagement, scheduling, and care coordination activities
- Monitor market performance metrics including utilization, care interventions, and quality outcomes
- Present clinical and operational performance results to executive leadership
- Supervise front-line clinical and operational staff within the assigned market
- Collaborate with finance and medical economics teams to address utilization trends and improve care delivery
- Develop strategies to reduce hospital admissions and readmissions for high-risk patient populations
- Support care transitions and post-discharge care management activities
- Provide clinical guidance to teams to support population health management and risk-based care models
- Participate in on-call rotation and provide coverage for other physician leaders as needed
Required Qualifications:
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited medical school
- Board Certified or Board Eligible in an ACGME-recognized specialty (such as Internal Medicine, Nephrology, Family Medicine, Emergency Medicine, Critical Care, Cardiology, Endocrinology, Hepatology, or Geriatrics)
- Active and unrestricted state medical license
- Willingness to obtain additional state licenses as required within the market
- Demonstrated experience applying evidence-based clinical criteria
- Experience managing high-need patient populations including Medicare Advantage and Medicaid
- Experience with quality improvement, utilization management, and value-based care models
- Strong leadership, communication, and team management skills
- Knowledge of healthcare quality frameworks such as NCQA, HEDIS, and risk adjustment models
- Experience working in renal care, geriatrics, or complex chronic disease management
