$120,000 - $145,000

Job Overview:
We are seeking a Palliative Care Nurse Practitioner to join a multidisciplinary care team delivering in-home care to patients with complex chronic conditions. This role focuses on providing compassionate, patient-centered palliative care to individuals living with advanced illnesses such as CKD, ESRD, diabetes, COPD, and CHF.

The Nurse Practitioner will work closely with the Market Physician Executive and an interdisciplinary care team to support symptom management, advance care planning, and improved quality of life for patients and their families. This role emphasizes population health management, shared decision-making, and coordinated care delivery for medically complex patients.

Key Responsibilities:

  • Collaborate with the Market Physician Executive and clinical leaders to identify patients who qualify for palliative care services
  • Build and manage a patient panel of individuals with complex chronic conditions
  • Develop individualized care plans based on evidence-based interventions and care pathways
  • Provide compassionate palliative care primarily through in-home patient visits, with virtual care as needed
  • Evaluate and treat symptoms that affect quality of life, including pain, pruritus, depression, anxiety, and insomnia
  • Collaborate with pharmacists and other clinicians to manage medication plans for complex patients
  • Educate patients and families on disease progression, treatment options, and palliative care principles
  • Facilitate advance care planning conversations and document patient wishes using AD/POLST forms
  • Support patients and families in navigating complex medical decisions and understanding prognosis
  • Coordinate care with interdisciplinary team members including social workers, chaplains, and other clinicians
  • Assist with transitions to hospice care when appropriate
  • Participate in clinical leadership meetings to support program performance goals and KPIs
  • Educate and support care team members on palliative care practices, symptom management, and communication

Required Qualifications:

  • Master's degree in Nursing
  • Board certification as a Nurse Practitioner
  • Active and unrestricted state licensure
  • Willingness to obtain additional state licenses as required
  • Minimum of 2 years of experience in a palliative care setting
  • Reliable transportation and ability to travel locally for in-home patient visits

Preferred Qualifications:

  • ACHPN or CHPN certification
  • Experience performing clinical assessments and developing evidence-based care plans for complex patients
  • Strong communication skills for sensitive conversations regarding prognosis and end-of-life care
  • Experience documenting complex cases within electronic health record (EHR) systems
