Salary Range: $67,000/year - $90,000/year (dependent upon experience)
Job Overview:
We are seeking a dedicated and skilled Physical Therapist (PT) to join our team. The PT will evaluate and treat patients across the lifespan, from pediatric to geriatric populations, in accordance with established policies and procedures. This position plays a critical role in helping patients/residents regain mobility, reduce pain, and improve their overall quality of life. The Physical Therapist will collaborate with the interdisciplinary team to deliver patient-centered care.
Key Responsibilities:
- Evaluate patients/residents to assess their needs and develop individualized treatment plans.
- Establish realistic, measurable short-term and long-term goals in collaboration with the referring provider, patient, family, caregivers, and other team members.
- Provide therapy treatments and evaluate their effectiveness according to department policies and procedures.
- Educate patients, families, caregivers, and team members to enhance the achievement of therapy goals.
- Document treatment and patient progress accurately and in a timely manner, according to department policies.
- Communicate effectively with team members, referring providers, and case managers regarding patient status, progress, needs, and discharge planning.
- Delegate appropriate treatments to therapy assistants and provide necessary supervision and direction.
- Complete reevaluations, daily billing, and patient scheduling efficiently.
- Participate in continuing professional development and department/hospital-wide educational opportunities.
- Contribute to departmental projects, performance improvement initiatives, or hospital committees.
- Adhere to all hospital and Rehab Department safety policies and procedures, reporting any incidents as required.

Qualifications:
- Education: Bachelor's Degree in Physical Therapy from an accredited program required.
- Experience: 1-2 years of experience preferred.
- Licensure: Must be licensed as a Physical Therapist in the state of New Hampshire (active at time of submission).
- Certifications:
- BLS (Basic Life Support) required (American Heart Association).
