Job Description:
As a Clinical Case Manager, you will be responsible for planning, organizing, and coordinating swing bed admissions, reviewing emergency department admissions and discharges, and managing medical records. This role is pivotal in ensuring smooth transitions for patients, working closely with the Director of Nursing. Your focus will include case management, social services, utilization review, and discharge planning while also collaborating with an interdisciplinary team to provide exceptional patient care.
Key Responsibilities:
- Perform financial assessments to determine patients' payment sources for in-patient stays.
- Provide information in response to queries from doctors' offices, families, and external facilities.
- Manage case management, social services, utilization review, and discharge planning.
- Identify and coordinate swing bed admissions, ensuring admission requirements are met.
- Review ED admissions/discharges and coordinate follow-up visits with primary care providers.
- Collaborate with specialty offices for patient transfers or care coordination.
- Participate in interdisciplinary team meetings and utilization review (UR) meetings.
- Review patient charts and assess for counseling or community resources as needed.
- Must be available at least two Saturdays a month for admissions.
Knowledge, Skills, and Abilities:
- Bilingual in Spanish preferred.
- Strong multitasking abilities.
- Proficiency with Microsoft Office, Athena, EMR, and Point Click Care software systems.
- Ability to work effectively in a fast-paced environment.
- Problem-solving skills with a focus on improving patient care.
- Ability to educate patients and families about disease processes and treatments.
- Strong communication skills to coordinate patient care plans across departments.
- Knowledge of community resources and support services.
Qualifications:
- Three to five years of experience in clinical nursing.
- One to two years of experience in case management or related field preferred.
- Current licensure to practice nursing in the state of California.
- Bachelor's degree in nursing highly preferred.
- BLS required, ACLS preferred.
