Position Title: Director of Case Management

Salary: $146,000+ annually, with potential for a 15% bonus incentive

Sign-On Bonus: $10,000

Relocation: Considered on a case-by-case basis

Schedule: Full-time, 40 hours per week with variable shifts

Job Summary:

We are seeking a Director of Case Management to lead and coordinate activities across our dynamic case management department. This pivotal role oversees staff compliance with organizational and external regulations, manages patient care integration, and ensures continuity and efficiency of care across various departments and external agencies.

Responsibilities:

  • Oversee daily operations of the case management department, ensuring alignment with hospital goals and compliance with regulatory standards.
  • Manage resource allocation to optimize staff productivity and meet quality objectives.
  • Spearhead initiatives to enhance care coordination and discharge planning processes, ensuring timeliness and accuracy in documentation.
  • Collaborate with senior financial and clinical leaders to align departmental objectives with broader organizational goals.
  • Evaluate and enhance department performance through data analysis and the implementation of sustainable improvement strategies.
  • Establish and maintain effective relationships with key stakeholders, including CMO, CNO, and other department heads.
  • Ensure staff development and uphold high standards of professionalism and ethics.

Qualifications:

  • Required: Bachelors degree in Nursing.
  • Preferred: Masters degree in Nursing, Health Administration, or Business Administration.
  • Minimum 3 years of acute care hospital case management experience.
  • At least 2 years in leadership within case management in an acute care setting.
  • Strong preference for candidates with previous experience in an HCA facility.
  • Excellent leadership, communication, and organizational skills.
  • Proven ability to manage multiple priorities in a high-pressure environment.

  • Direct reports include 2 Case Management Managers and a team of 30 Case Management Staff.
