Position Summary:
The Physical Therapist in Home Health is responsible for evaluating patients and applying a variety of therapeutic interventions to rehabilitate physical and developmental disorders. This role will coordinate care and promote patient self-determination to achieve the highest level of independence.
Qualifications:
- Bachelors degree in Physical Therapy required.
- Doctorate of Physical Therapy preferred.
- Less than one year of physical therapy experience required.
- Licensed Physical Therapist required.
- BLS Certification required.
- Proficiency in Microsoft Office.
- Ability to provide evidence-based care and support achieving optimal outcomes.
- Ability to foster a compassionate and therapeutic environment.
- Utilizes clinical reasoning, critical thinking, and judgment in patient care.
- Engages in collaborative care with multidisciplinary teams and patient families.
- Promotes self-care and education to patients and their significant others.
- Advocate for patients in resolving clinical or ethical issues.
Responsibilities:
- Evaluate and treat patients regarding various therapeutic interventions for physical function and developmental disorders.
- Oversee and ensure a comprehensive rehabilitation program to meet the physical and psychosocial well-being of the patient.
- Collaborate with various disciplines involved in patient care.
- Set discharge goals and coordinate care to meet them.
- Advocate for patients and optimize resource utilization for the highest level of patient independence.
- Ensure knowledge of patient growth and development principles.
- Perform additional duties as required.
