Position Title: Registered Nurse Care Manager - Clinical Diabetes Educator

Location: Bethel, Alaska

Salary: Competitive, plus a $30,000 sign-on bonus with a three-year agreement

Position Summary:

This grant-funded position involves providing comprehensive diabetes education and care for patients with Type 1, Type 2, gestational diabetes, and those at high risk of developing diabetes. Education is delivered through telephonic communication, video teleconferencing, in-person sessions, and may require travel to remote village clinics. The RN Case Manager/Clinical Diabetes Educator will manage patient care, order labs, document in the electronic health record, and collaborate closely with village and hospital staff. The role includes participating in outreach activities and promoting diabetes standards of care.

Key Responsibilities:

  • Provide comprehensive diabetes education for patients with diabetes or at risk of developing diabetes.
  • Deliver care and education via telecommunication, video, and in-person sessions, including village travel.
  • Order lab tests according to hospital policies and document education in electronic health records.
  • Plan and coordinate diabetes-related health promotion activities in the YK Delta Region.
  • Maintain and update the Diabetes Registry and fulfill grant requirements.
  • Collaborate with village clinics, hospital staff, dietitians, and pharmacists to ensure comprehensive patient care.
  • Participate in outreach events such as diabetes support groups, radio shows, and media campaigns.
  • Engage in department planning and goal setting to improve patient care and support partnerships with tribal leaders.

Qualifications:

  • Completion of a nursing education program from an NLN-approved institution.
  • Minimum of an Associate's Degree in Nursing (BSN or MSN preferred).
  • New graduates with a valid RN license may be considered.
  • At least two years of clinical health experience preferred.
  • Background in Diabetes Education is preferred.
  • Valid RN license in Alaska required.
  • Must obtain CDCES certification within two years.
  • BLS certification preferred.
  • Strong knowledge of diabetes, cardiovascular disease, and public health.
  • Proficiency in using diabetes care medical equipment and PC computers.
  • Ability to travel to remote villages via small planes and work in sub-arctic conditions.
  • Cross-cultural communication skills essential; Yupik/English bilingualism is preferred.
