Position Title: Director of Case Management

Salary Range: $102,107 - $153,088

Schedule: Full-time, 36 hours per week, variable shifts

Job Overview:

We seek a dynamic Director of Case Management to oversee and coordinate the operational activities of our case management department. The role requires a strategic leader capable of ensuring seamless integration of patient care and discharge planning processes across multiple departments and external agencies, enhancing continuity of care, and optimizing clinical resource utilization.

Job Responsibilities:

  • Leadership and Oversight: Provide leadership and management to the case management team, ensuring alignment with organizational policies and external regulatory requirements.
  • Strategic Planning: Coordinate with hospital leadership to implement strategic initiatives aimed at enhancing patient outcomes and departmental efficiency.
  • Resource Management: Allocate and manage departmental resources effectively to meet productivity and quality goals.
  • Performance Improvement: Evaluate and enhance the departments performance through data analysis and implementation of sustainable improvement practices.
  • Collaboration: Establish and maintain productive relationships with key internal stakeholders, including CMO, CNO, and ancillary service leaders, as well as external partners.
  • Compliance and Quality: Ensure adherence to all relevant policies and standard operating procedures, focusing on care coordination and discharge planning efficacy.

Qualifications:

  • Education: Bachelor's degree required; Master's degree in Nursing, Health Administration, or Business Administration preferred.
  • Licensure: Must hold a current RN license or LCSW, with eligibility in the state of practice.
  • Experience: Minimum of 5-7 years in acute care hospital case management and at least 2+ years in a leadership role within a large acute care or trauma-level facility.
  • Skills: Excellent communication, presentation, and interpersonal skills are essential. Must be a proven leader with the ability to manage multiple priorities, initiate and manage change, and build effective teams.
