Job Overview:

The Hospice Registered Nurse works under the supervision of the unit Supervisor/Manager/Director and is responsible for delivering compassionate care to hospice patients. The nurse will assess patient needs, develop care plans, and provide guidance and education to patients and their families. This role requires leadership skills, flexibility, and strong communication abilities. The Hospice RN must adhere to professional nursing standards and regulatory requirements, while consistently contributing to an excellent patient and family experience.

Key Responsibilities:

  • Perform comprehensive physical, psychological, and social assessments of hospice patients on admission and document within required timeframes.
  • Develop and implement personalized care plans based on patient needs, revising as necessary.
  • Provide emotional, psychological, and spiritual support to patients and their families, fostering a compassionate care environment.
  • Promote patient independence through education on their condition, medications, and self-care skills.
  • Maintain high standards of patient care in compliance with hospital policies, state nursing board regulations, and nursing practice standards.
  • Communicate effectively with the multidisciplinary team, patients, and families to ensure continuity of care.
  • Identify changes in patient conditions and take appropriate actions to address them.
  • Ensure compliance with infection control, medication storage, and controlled substance regulations.
  • Participate in team problem-solving, build rapport among healthcare teams, and engage in leadership through delegation and communication.
  • Provide ongoing patient and family education related to disease processes, care requirements, and end-of-life expectations.

Qualifications:

  • Graduate from an accredited nursing school or anticipated graduation within 4 months of hire date.
  • Active RN license in Michigan.
  • BLS Certification (required).
  • At least 2 years of RN experience (required).
  • Hospice experience (strongly preferred).
  • Must have reliable transportation and a valid drivers license.
