Job Type: Full-Time

Shift: Varies; Monday-Friday 8 hours. Must be available at least two Saturdays a month depending on admissions

Salary: $150,000.00 to $165,160.00

Housing: Studio apartment offered at $200 per week (paid by employee) with a $200 non-refundable cleaning fee.

Job Overview:

The Clinical Case Manager will play a key role in planning, organizing, and coordinating swing bed admissions, reviewing emergency department admissions and discharges, and overseeing case management and discharge planning. This individual will be responsible for coordinating medical records, providing case management services, and facilitating smooth transfers in collaboration with the interdisciplinary care team. Reporting directly to the Director of Nursing, the Clinical Case Manager must work effectively in a fast-paced environment.

Essential Responsibilities:

  • Perform financial assessments to determine patients' source of payment for in-patient services and begin the review process.
  • Respond to queries from doctors' offices, families, and outside facilities.
  • Provide case management, social services, utilization review, and discharge planning.
  • Identify and coordinate swing bed admissions and verify necessary admission requirements are met.
  • Review emergency department admissions and discharges, contacting patients to coordinate follow-up visits or facilitate transfers.
  • Develop and maintain relationships with specialty offices to assist with transfers or collaboration of care.
  • Attend interdisciplinary meetings and collaborate with the team to discuss patient care plans.
  • Educate patients and families about the disease process and treatment.
  • Be available at least two Saturdays per month depending on admissions.

Knowledge, Skills, and Abilities:

  • Bilingual in Spanish preferred.
  • Proficiency in Microsoft Office, Athena, EMR, and Point Click Care.
  • Ability to multitask and resolve patient care issues in a fast-paced environment.
  • Knowledge of community resources and support services.
  • Strong interpersonal and communication skills to inform and advise ancillary departments about changes in patient care plans.

Qualifications:

  • Education: Bachelor's degree in nursing is highly preferred.
  • Licensure: Current RN licensure to practice in the state of California required.
  • Experience: Three to five years of experience in clinical nursing, with one to two years of case management or related field preferred.
  • Certifications: BLS required, ACLS preferred.
