Job Overview
A well-established healthcare organization in the Adirondack region is seeking an experienced Clinical Pharmacist to join its Pharmacy Services team. This role is ideal for a pharmacist with a strong clinical background who is passionate about optimizing medication therapy, collaborating with healthcare providers, and delivering exceptional patient care in a hospital setting.
The Clinical Pharmacist will be responsible for the safe and effective dispensing of medications and pharmaceutical supplies while providing clinical support to physicians, nursing staff, and patients.
Responsibilities
- Review and verify medication orders for accuracy, appropriateness, and safety
- Dispense medications and pharmaceutical supplies in accordance with department policies and regulatory requirements
- Provide clinical pharmacy support and medication therapy recommendations
- Monitor patient medication regimens for effectiveness, interactions, and adverse effects
- Collaborate with physicians, nurses, and other healthcare professionals to optimize patient outcomes
- Counsel patients and families regarding medication use when appropriate
- Maintain accurate pharmacy records and documentation
- Ensure compliance with state, federal, and hospital regulations
- Participate in quality improvement and patient safety initiatives
Qualifications
Required
- Current New York State Pharmacist License
- Minimum of 2–3 years of clinical pharmacy experience
- Previous hospital pharmacy experience
- Strong understanding of medication management and clinical pharmacy practices
- Excellent communication and collaboration skills
Preferred
- Experience working in an acute care or inpatient hospital setting
- Strong clinical decision-making and patient care skills
