Salary: 
135,000 USD - 150,000 USD
Schedule: Monday–Friday, 8:00 AM–5:00 PM
Hybrid Role: home + virtual environment
Compensation: Base salary + Company bonus program

Job Overview:
We are seeking experienced Nurse Practitioners and Physician Assistants to join an integrated care team providing in-home and virtual care for patients with complex chronic conditions, primarily Chronic Kidney Disease (CKD) and End-Stage Renal Disease (ESRD/ESKD).

In this role, the Advanced Practice Provider collaborates with a Registered Nurse and Social Worker to deliver personalized, value-based care focused on improving patient quality of life and slowing disease progression. This position allows providers to spend meaningful time with patients while helping manage complex medical conditions in the home setting.

Key Responsibilities:

  • Conduct comprehensive patient assessments and annual wellness visits in patients' homes and through virtual care
  • Develop and implement individualized treatment plans based on patient needs and evidence-based care
  • Educate patients and families about disease management, treatment options, and preventive care
  • Coordinate care with primary care providers, specialists, and other healthcare partners
  • Prescribe medications, order diagnostic tests, and collaborate with supervising physicians
  • Maintain accurate and timely clinical documentation including medical history, assessment, and care plans
  • Monitor patient progress and adjust treatment plans as necessary
  • Support closing gaps in care through preventive monitoring and coordination of diagnostic testing
  • Participate in interdisciplinary care team meetings
  • Provide patient education and resources to promote healthier lifestyles and disease management
  • Deliver care focused on reducing hospitalizations and improving long-term health outcomes

Required Qualifications:

  • Active and unrestricted Nurse Practitioner or Physician Assistant license
  • Board certification (NP: ANCC or AANP; PA: NCCPA)
  • Current DEA certification or eligibility to obtain
  • Minimum of 2 years of direct patient care experience
  • Ability to practice independently and work in a field-based environment
  • Valid driver's license, reliable transportation, and car insurance
  • Proficiency with medical technology, electronic health records, and clinical documentation systems
  • Strong communication skills when working with patients, families, and healthcare partners
  • Knowledge of federal and state healthcare regulations including HIPAA and OSHA

Preferred Qualifications:

  • Bilingual in English and Spanish
  • Managed care, IPA, or health plan experience
  • Experience conducting annual wellness visits or comprehensive patient evaluations in-home or virtually
