Job Title: Nurse Practitioner
Location: must live within 20 miles of Kingman, AZ
Salary: $120,000 - $135,000
Job Overview:
The Advanced Practice Provider will deliver personalized, compassionate medical care to patients with chronic kidney disease and end-stage renal disease. This is a field-based role focused on in-home and virtual comprehensive assessments, care coordination, and preventative management within a value-based care model. The provider will work closely with field nurses, community health workers, and physicians to improve patient outcomes, reduce avoidable hospitalizations, and enhance quality of life.
Key Responsibilities:
- Conduct comprehensive annual wellness visits in the home and virtual settings
- Perform health assessments, develop treatment plans, and evaluate patient progress
- Prescribe medications, order diagnostic tests, and collaborate with supervising physicians
- Educate patients and families on disease management and preventative care
- Coordinate care with primary care providers, specialists, and ancillary services
- Close gaps in care by facilitating diagnostic testing and preventive monitoring
- Document comprehensive medical records including history, diagnoses, medications, and care plans
- Participate in integrated care team meetings
- Deliver evidence-based care focused on quality outcomes and patient satisfaction
- Assist patients with accessing educational resources
Required Qualifications:
- Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Associate license
- Board certification (NP: ANCC or AANP; PA: NCCPA)
- Current and unrestricted DEA certificate
- Minimum of 2 years of direct patient care experience
- Must be bilingual in English and Spanish
- Must live within 20 miles of San Luis, Arizona
- Valid driver's license, reliable transportation, and car insurance
- Ability to practice autonomously without direct supervision
- Strong knowledge of HIPAA, OSHA, and federal and state regulations
- Proficiency with electronic medical records and point-of-care technology
- Managed Care, IPA, or Health Plan experience
- Experience conducting annual wellness visits in the home or virtually
- Experience in chronic kidney disease management or value-based care environments
