Responsibilities
Medication Reconciliation
Review and verify patients medication histories for accuracy and completeness.
Collaborate with pharmacists and healthcare teams to resolve discrepancies.
Patient Education & Support
Educate patients on medication usage, potential side effects, and adherence strategies.
Address patient concerns or connect them with the appropriate provider.
Medication Adherence Monitoring
Utilize electronic health records (EHR) and other tools to track medication adherence.
Proactively follow up with patients at risk of non-adherence and report concerns to the care team.
Care Coordination
Assist with care transitions, ensuring seamless medication management during hospital discharges or setting changes.
Coordinate medication refills, prior authorizations, and delivery services.
Data Entry & Documentation
Maintain up-to-date, accurate patient records in the EHR system.
Document patient interactions and interventions to support continuity of care.
Support for Healthcare Teams
Work with pharmacists to identify potential drug interactions or contraindications.
Provide data and insights to enhance care plans and improve patient outcomes.
Compliance & Quality Assurance
Ensure compliance with HIPAA, state pharmacy regulations, and industry standards.
Participate in quality improvement initiatives to enhance medication management and patient safety.
Qualifications
Required:
Certified Pharmacy Technician (CPhT) credential from a recognized certifying body (e.g., PTCB or NHA).
Previous experience in a pharmacy setting, preferably with a focus on care management.
Strong knowledge of pharmacology, medication therapy management (MTM), and patient-centered care.
Skills:
Proficiency with EHR systems, pharmacy software, and telehealth tools.
Excellent verbal and written communication skills for patient and provider interactions.
Strong attention to detail for accurate documentation and medication management.
Problem-solving mindset with a proactive approach to addressing medication-related issues.
