A Pharmacy Technician in Care Management plays a crucial role in supporting healthcare teams to ensure patients receive safe and effective medication therapy. This position involves working closely with pharmacists, nurses, and physicians to manage patient medication needs efficiently.

Key Responsibilities

Medication Reconciliation

Review and document patients' medication histories with accuracy.

Identify and resolve discrepancies in medication lists with the pharmacist.

Patient Support & Education

Educate patients on proper medication use, potential side effects, and adherence.

Address patient concerns or refer them to the appropriate healthcare provider.

Medication Adherence Monitoring

Track patient adherence using electronic health records (EHR) and pharmacy tools.

Follow up with patients struggling with adherence and report concerns to the care team.

Care Coordination

Communicate with healthcare providers to support care transitions, such as hospital discharges.

Assist with medication refills, prior authorizations, and delivery coordination.

Data Entry & Documentation

Accurately update patient records in EHR systems.

Document interactions with patients and healthcare teams.

Support for Pharmacists & Healthcare Teams

Assist in medication reviews, identify potential drug interactions, and support medication safety initiatives.

Provide reports on medication usage to aid in care planning.

Compliance & Quality Assurance

Ensure adherence to HIPAA, state pharmacy laws, and regulatory requirements.

Participate in quality improvement initiatives to enhance patient care.

Required Skills & Qualifications

Certification: Certified Pharmacy Technician (CPhT) through PTCB or NHA.

Experience: Prior experience in a pharmacy setting, preferably in care management.

Knowledge: Strong understanding of pharmacology, medication therapy management (MTM), and chronic disease management.

Technical Skills: Familiarity with EHR systems, pharmacy software, and telehealth tools.

Communication: Excellent verbal and written skills to effectively collaborate with patients and healthcare teams.

Attention to Detail: Accuracy in documentation and medication processing.

Problem-Solving: Ability to identify and resolve medication-related concerns with the care team.

A background check is required for this role.

Pay: $18.00 - $23.00 per hour
