Certified/Licensed Pharmacy Technician

The Pharmacy Technician in Care Management assists healthcare teams in managing patients' medication therapy, collaborating with pharmacists, nurses, and physicians.

Key Responsibilities:

Review and document medication histories, resolving discrepancies.

Educate patients on medication use, side effects, and adherence.

Monitor and follow up on medication adherence.

Coordinate care, including refills, authorizations, and care transitions.

Accurately update patient data and document interactions.

Support medication reviews and care plan development.

Ensure compliance with regulations and participate in quality improvement.

Qualifications:

Certified Pharmacy Technician (CPhT) or licensed.

Previous pharmacy or care management experience.

Strong knowledge of pharmacology and medication therapy management.

Proficient in EHR systems and pharmacy software.

Excellent communication and attention to detail.

Background check required.
