I hereby authorize Menicon Singapore Pte Ltd (MSPL) and/or Menicon Singapore Sales Pte Ltd (MSS), along with any third-party service provider appointed for this purpose, to conduct background checks on my previous employment and make related inquiries as part of the review and assessment of my employment application.

I understand that the collection, use, and disclosure of my personal data will be used to verify the accuracy of the information I provided to MSPL/MSS in my application form and/or curriculum vitae, as deemed necessary at the Company’s discretion.

Furthermore, I consent to the collection, use, and disclosure of my personal data, including the compilation and/or reproduction of any information obtained, into reports or documents (whether in written or electronic formats) for MSPL/MSS’s reference and consideration.

I acknowledge that this consent will remain valid until I withdraw it in writing. I understand that I may withdraw my consent at any time, with reasonable notice to MSPL/MSS.

I also authorize any person(s) contacted during these inquiries to disclose relevant information to MSPL/MSS and release all such persons from any liability associated with this disclosure, whether under the Personal Data Protection Act 2012, general rules on confidentiality, or other applicable laws or regulations.

I further consent to a photocopy of this authorization being accepted with the same authority as the original.
