Saint Francis Hospital in Evanston, Illinois, part of the Prime Healthcare network, is currently seeking a dedicated and compassionate Primary Care Nurse Practitioner to join our outpatient clinic located on Peterson Avenue in Chicago. Position Details
- Full-time employment in an outpatient primary care clinic
- Spanish-speaking preferred to support the needs of the local population
- Clinic is an extension of Saint Francis Hospital Evanston, offering seamless coordination of care
- Deliver full-spectrum primary care to patients of all ages
- See patients for preventive care, chronic disease management, and acute visits
- Collaborate closely with a team of physicians, APPs, and clinical support staff
- Manage your own patient panel with opportunities for continuity of care
- 40-hour work week with minimal call responsibilities
- Participate in care coordination and hospital-connected initiatives that elevate community health outcomes
- Maintain accurate and timely documentation using the electronic medical record (EMR)
Qualifications
- Licensed and certified as a Nurse Practitioner (NP) in the state of Illinois
- Board Certification in Family Medicine (NP) or appropriate PA certification
- Spanish proficiency strongly preferred
- Prior experience in Primary Care preferred but not required
- Strong clinical assessment, communication, and documentation skills
- Passion for delivering high-quality, compassionate care in a community-focused setting

A reasonable compensation estimate for this role, which includes estimated wages, benefits, and other forms of compensation, is $110,000.00 to $125,000.00 on an annualized basis. The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire, in which a wide range of factors will be considered, including but not limited to, skillset, years of applicable experience, education, credentials and licensure.
