Holy Family Medical Center in Des Plaines, Illinois is currently seeking an experienced and compassionate Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our Primary Care team. This full-time opportunity offers a chance to deliver comprehensive outpatient care in a collaborative, patient-centered environment. Position Details

  • Full-time position in an outpatient primary care clinic
  • Polish language skills preferred to meet the needs of the local community
  • Open to Nurse Practitioners or Physician Assistants
  • See patients for preventive care, chronic disease management, and acute visits
  • Collaborate closely with a team of physicians, APPs, and clinical support staff
  • Manage your own patient panel with opportunities for continuity of care
  • 40-hour/week schedule with minimal call responsibility
  • Average of 16-24 patients per day
  • Supportive, mission-driven care environment

Qualifications

  • Licensed NP or PA in the state of Illinois
  • Board Certification in Family Medicine (NP) or appropriate PA certification
  • Minimum of 2 years of Primary Care or Family Practice experience preferred
  • New graduates with strong clinical rotations encouraged to apply

A reasonable compensation estimate for this role, which includes estimated wages, benefits, and other forms of compensation, is $110,000.00 to $135,000.00 on an annualized basis. The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire, in which a wide range of factors will be considered, including but not limited to, skillset, years of applicable experience, education, credentials and licensure.
