Job Description Summary

The Palliative Care Supervisor (PCS) functions as a clinical expert to manage and oversee a population of acute, complex, chronically and terminally ill patients. The PCS will oversee and administer preventive, palliative and chronic illness surveillance to this population. The PCS will oversee the clinical performance of the palliative care team, ensuring high standards of patient care and compliance with the Joint state Nurse Practice Act requirements. The PCS position will encompass various administrative duties to oversee and report on patients plan of care to medical groups and coordinating their plan of care with designed palliative or hospice team.

This is an onsite position based at our Santa Ana office, with occasional field work and visits.

MINIMUM REQUIREMENTS

1. Registered Nurse in the State with current licensure to practice professional nursing in the State.

2. At least two (2) years previous experience clinical practice in hospice, palliative care, chronic care management

3. Management skills, expertise and availability to manage patients in different post-acute environments.

4. Three years of management experience within the last five years in hospice or related health care organizations, at least one year of which was a supervisory or administrative capacity.

5. Demonstrated ability to supervise and direct professional and administrative personnel.

6. Ability to market and deal tactfully with customers and the community.

7. Has excellent observation, verbal and written communication skills.

PREFERRED

1. Current Board Certification in Hospice and Palliative Medicine is highly desirable.

2. At least five (5) years of clinical practice in hospice, palliative care, chronic care management.

3. Bilingual skill in Spanish preferred.

Job Type: Full-time

Pay: From $110,000.00 per year (exempt position)

Schedule:

  • 8 hour shift
  • Day shift
  • Monday to Friday
  • Weekends as needed

  • Supervisory: 1 year (Preferred)
