Saint Joseph Medical Center is currently seeking an experienced and compassionate Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our Primary Care team in Plainfield, Illinois. This is a full-time opportunity to provide high-quality outpatient care in a collaborative, patient-centered environment.

Position Details

  • Full-time employment in an outpatient primary care clinic
  • Open to Nurse Practitioners or Physician Assistants
  • Deliver full-spectrum primary care to patients of all ages
  • Manage chronic conditions, perform preventative care, and address acute health concerns
  • Collaborate closely with a team of physicians, APPs, and clinical support staff
  • Work closely with physicians and clinical staff to ensure coordinated, high-quality care
  • Maintain accurate and timely documentation in the electronic medical record
  • Manage your own patient panel with opportunities for continuity of care
  • Monday-Friday schedule with minimal call responsibility
  • Average of 16-24 patients per day
  • 36 hours of patient contact per week
  • 4 administrative hours weekly

Qualifications

  • Licensed NP or PA in the state of Illinois
  • Board Certification in Family Medicine (NP) or appropriate PA certification
  • Minimum of 2 years of Primary Care or Family Practice experience preferred
  • New graduates with strong clinical rotations are encouraged to apply

A reasonable compensation estimate for this role, which includes estimated wages, benefits, and other forms of compensation, is $110,000.00 to $135,000.00 on an annualized basis. The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire, in which a wide range of factors will be considered, including but not limited to, skillset, years of applicable experience, education, credentials and licensure.
