Compensation: $100,000 $112,000 annually, plus eligibility for quarterly bonus

An excellent opportunity is available for an experienced leader to step into the role of Director of Case Management at a 60-bed acute care hospital with an average daily census of 4648 patients in 2024.

Position Overview

The Director of Case Management oversees a dynamic team of 7 direct reports, including 4 full-time case managers (2 RNs and 2 social workers), 1 part-time case manager assistant, and 2 per diem case managers. This leader is responsible for advancing patient-centered case management, utilization review, and discharge planning practices while ensuring compliance with state and federal regulations.

This role requires a leader who is results-driven, collaborative, and able to foster effective communication across clinical teams to ensure optimal patient outcomes and efficient transitions of care.

Key Responsibilities

  • Direct and manage all case management operations including utilization review, discharge planning, and care coordination.
  • Mentor and develop a diverse team of case managers and support staff.
  • Partner with clinical and administrative leadership to improve patient flow, outcomes, and resource utilization.
  • Monitor performance metrics and implement process improvements to meet quality and regulatory standards.

Candidate Profile

  • RN preferred; must hold a current state license (RN or Social Worker).
  • If nursing: Bachelors degree in Nursing (BSN) required.
  • If Social Worker: Masters degree in Social Work required.
  • Certification required: CCM or ACM, to be obtained within 12 months of hire.
  • Experience: Minimum of 3 years in hospital-based case management, including utilization review and discharge planning.
  • Proven leadership skills with ability to manage teams in an acute care environment.
