We are seeking an experienced and motivated **Director of Care Management** to join our healthcare team in Fresno, CA. This is a strategic leadership role that oversees care coordination, discharge planning, and utilization management to ensure high-quality, patient-centered care across the continuum. The Director of Care Management will work collaboratively with clinical teams, physicians, and support staff to enhance patient outcomes, optimize resources, and promote efficiency.
**Key Responsibilities:**
- Develop, implement, and oversee care management processes to support quality improvement and effective patient care coordination.
- Lead a team of case managers and social workers, providing guidance, mentorship, and professional development.
- Ensure compliance with all healthcare regulations, accreditation standards, and organizational policies.
- Collaborate with clinical staff, physicians, and administrative teams to streamline transitions of care, discharge planning, and patient advocacy.
- Analyze data to monitor performance metrics, identify trends, and drive continuous improvements in care management services.
- Manage budget and resources for the care management department, ensuring operational efficiency.
- Foster strong communication and partnerships within and outside the organization to support patient needs and organizational goals.
**Qualifications:**
- Must be a licensed **Registered Nurse (RN)** in California.
- Bachelors degree in Nursing required; Masters degree in Nursing, Healthcare Administration, or a related field preferred.
- Minimum of 5 years of experience in care management or case management, with at least 2 years in a leadership role.
- Strong knowledge of healthcare regulations, discharge planning, and utilization review.
- Excellent interpersonal, communication, and organizational skills.
