As a Social Worker within the Senior Services team, you will work to promote social-health wellbeing of seniors by engaging them meaningfully

Roles and Responsibilities:

• Conduct triage and intake services for members of the community.
• Provide case management to individuals and families.
• Conduct home visits to assess clients' needs and create personalized care plans.
• Perform psycho-social assessments to recommend suitable service linkages.
• Collaborate with partners to coordinate seamless care.
• Maintain accurate and organized records and files
• Assist with company projects or report submissions.
• Participate in internal and external training, supervision and team meetings.
• Carry out any other duties as assigned by HOD and supervisor.

Candidate Profile:

• Bachelor / Postgraduate Diploma / Master in Social Work.
• No prior work experience required for new entrants to social work.
• Passionate to work with seniors and their caregivers in community setting
• Ability to work independently and as part of a team.
• Strong communication and organizational skills.
• Effective written and oral communication in English
• Fluency in Mother Tongue language and/or dialects will be advantageous
• Commitment to the Code of Social Work Practice standards.
