Home Health Admissions RN (Start of Care Focus)

4-Day Workweek | $80K + $10K Bonus

If you already work in home health and enjoy the admissions side of care — this role is structured specifically around Start of Care (SOC) visits.

This is not a hospital-to-home transition role. Prior Home Health RN experience is required.

Schedule:
Monday–Thursday, 8am–6pm OR Tuesday–Friday, 8am–6pm

Territory:

Snyder, Union, Lycoming, Clinton, and Upper Northumberland Counties

What this role requires:

• This is a field-based role requiring reliable transportation and comfort driving within assigned counties.
• Expectation: 3 Start of Care admissions per day
• No built-in weekend rotation listed for this position.
• Active RN license in Pennsylvania
• Prior Home Health RN experience (required)
• Valid drivers license
• BLS (ACLS acceptable as well)
• Comfort with EMR systems

Base salary: 

Approximately $80,000

$10,000 First-Year Success Bonus

This position is built for an RN who:

• Is already comfortable completing independent SOC assessments
• Can manage 3 admissions per day efficiently
• Is disciplined with same-day or near same-day documentation
• Is confident using tablet/mobile EMR systems
• Self-manages scheduling and time in the field
• Strong assessment skills and workflow discipline are essential.
