The role

• Manage and deliver patient care plans in a home setting

• Work independently while supported by a clinical team

• Coordinate care and complete documentation

• Travel locally between patient visits (~20% driving)

Schedule

• Weekend-focused role:

  • Saturday & Sunday patient visits

  • On-call Friday, Saturday, Sunday evenings (5pm–10pm)

• 2 holidays per year

What they’re looking for

• Active RN license (PA)

• 1+ year RN experience

• 1+ year of clinical documentation, clinical informatics, OR clinical quality

• 1+ year of EMR, Other EMR, OR practice partner EMR

• Comfortable working independently and managing your own schedule

• Experience with EMR/documentation systems

• Associate degree or higher

• Okay with the required local travel

Certifications:

• AHA ACLS or ARC ALS

Compensation

• Estimated earnings: $77K–$100K+ (based on productivity)

• Per visit: $42–$46

• $10K sign-on bonus

• Quarterly bonuses

• 6-month guaranteed income support
