The role

As a Hospice RN Case Manager, you'll manage and coordinate care for patients in a home-based setting, working closely with an interdisciplinary team to ensure comfort, dignity, and quality of life.

You'll be responsible for:

• Managing patient care in hospice settings

• Coordinating with physicians and care teams

• Supporting patients and families through all stages of care

• Providing leadership and guidance within the care team

Schedule & structure

• Monday–Friday schedule (daytime hours with flexibility in start/end times)

• Limited on-call expectations:

  • 1 weekend day per month (scheduled visits + overnight on-call)

  • 2 weekday on-call shifts per month

• Dedicated after-hours RN team in place for additional support

• Additional compensation provided for on-call coverage

• 2 holidays per year + floating holidays

Territory

• Local travel within a defined service area

• Focus on maintaining reasonable coverage and minimizing unnecessary travel

What they're looking for

• Active RN license in Illinois

• 1+ year of hospice or home health experience

• Experience in case management or care coordination

• Strong clinical judgment and ability to work independently

• A passion for patient-centered, end-of-life care

Certifications:

• AHA BLS (required)

• AHA ACLS (required)

Compensation

• $70,000 - 90,000 annually

• Additional stipends for on-call coverage
