The role
As a Hospice RN Case Manager, you'll manage and coordinate care for patients in a home-based setting, working closely with an interdisciplinary team to ensure comfort, dignity, and quality of life.
You'll be responsible for:
Managing patient care in hospice settings
Coordinating with physicians and care teams
Supporting patients and families through all stages of care
Providing leadership and guidance within the care team
Schedule & structure
Monday–Friday schedule (daytime hours with flexibility in start/end times)
- Limited on-call expectations:
1 weekend day per month (scheduled visits + overnight on-call)
2 weekday on-call shifts per month
Dedicated after-hours RN team in place for additional support
Additional compensation provided for on-call coverage
2 holidays per year + floating holidays
Territory
Local travel within a defined service area
Focus on maintaining reasonable coverage and minimizing unnecessary travel
What they're looking for
Active RN license in Illinois
1+ year of hospice or home health experience
Experience in case management or care coordination
Strong clinical judgment and ability to work independently
A passion for patient-centered, end-of-life care
Certifications:
AHA BLS (required)
AHA ACLS (required)
Compensation
$70,000 - 90,000 annually
Additional stipends for on-call coverage
Location
Openings available in McHenry, Mount Pleasant, and Northbrook, IL
