This opportunity is designed for an experienced RN who wants ownership of their patients, a structured caseload, and a team that actually supports the work behind the scenes.
You'll be working within a defined territory around Canonsburg and surrounding communities, allowing you to focus on patient care without excessive travel.
What You Can Expect
- Structured caseload: ~6 patient visits per day / 30 units per week
- Consistent weekday schedule: Monday–Friday, 8 AM – 5 PM

Predictable on-call rotation:
- 1 weekend per month
- 1 evening per week
- Defined territory: Canonsburg + ~15–25 mile radius
- Team-based hospice model with clinical support
The Role
- Manage and coordinate hospice patient care across your assigned territory
- Serve as the primary point of contact for patients and families
- Collaborate with interdisciplinary teams to adjust care plans as needed
- Provide guidance and support to other nursing staff when appropriate
- Maintain timely and accurate documentation

What You Bring
