Director of Dementia Care – At Home Harmony (Richmond, VA)

At Home Harmony is a rapidly growing Richmond-based healthcare organization transforming care for complex, homebound seniors. Our integrated model combines home-based primary care, long-term care pharmacy, dementia services, and caregiver support to help older adults remain safely and comfortably at home for as long as possible.
We are seeking an experienced, mission-driven Director of Dementia Services to lead and grow our participation in the CMS Guiding an Improved Dementia Experience (GUIDE) Model.
THE OPPORTUNITY
This is a high-impact leadership role for someone who understands dementia care and enjoys turning vision into action. You will have the opportunity to build, lead, and expand a comprehensive dementia-care program while improving the experience of both patients and their caregivers.
You will oversee program operations, interdisciplinary teams, quality and compliance, community partnerships, caregiver support, and continued growth of the GUIDE program.
WHAT YOU'LL DO
- Lead the day-to-day operations and strategic growth of the GUIDE dementia program.
- Ensure compliance with CMS GUIDE requirements, quality measures, documentation, and reporting.
- Lead and develop an interdisciplinary team of clinicians, care navigators, social workers, and other care partners.
- Ensure patients and caregivers receive personalized education, care coordination, resources, and support.
- Develop strong relationships with community organizations, respite providers, senior living communities, adult day programs, and other referral partners.
- Monitor program performance, quality, enrollment, utilization, caregiver outcomes, and financial results.
- Identify opportunities for improvement and implement solutions that enhance both care delivery and operational performance.
- Recruit, train, coach, and develop team members.
- Collaborate with clinical, finance, billing, compliance, sales, and executive leadership.
- Help expand dementia services beyond the GUIDE population and grow the program throughout the At Home Harmony service area.
- Represent At Home Harmony and the GUIDE program within the community.
WHAT WE'RE LOOKING FOR
- Bachelor's degree in nursing, social work, healthcare administration, public health, business, or a related field; equivalent experience may be considered.
- Experience working with individuals living with dementia or Alzheimer's disease and their caregivers.
- Strong understanding of person-centered dementia care, caregiver needs, safety, behavioral and psychosocial issues, and community resources.
- Healthcare leadership or program-management experience with responsibility for teams, workflows, performance, and cross-functional collaboration.
- Experience with CMS, Medicare, value-based care, care management, or other regulated healthcare programs.
- Excellent communication, organization, analytical, and problem-solving skills.
- Ability to translate complex regulatory requirements into practical, effective workflows.
Preferred: Master's degree; clinical license (RN, LCSW, NP, PA, OT, PT, etc.); experience with the CMS GUIDE Model/CMMI; and experience in home-based primary care, geriatrics, memory care, long-term care, or community-based aging services.

Location: Henrico, VA — onsite with potential hybrid flexibility
Schedule: Full-time
Compensation: $105,000–$125,000 annually, commensurate with experience, certifications, and credentials
