You'll collaborate with a company that have personally visited and assessed hundreds of senior living options. They will walk with you step by step through the entire transition process, helping you feel educated and confident about your care options.
Work independently to schedule assignd patient caseload to the appropriate licensed clinical personnel and to support the common goals of the agency as it relates to the management and delivery of timely care to the ageny's patients
The main responsibilities of a SCHEDULING COORDINATOR include:
- Builds relationships with patients and field staff (RN, LVN, PT, PTA, OT, HHA and MSW).
- Actively manages available field staff to maximize visits per day based on coverage area and employment agreement.
- Plots patient visit frequencies and other discipline evaluations.
- Pre-triage field staff schedules- prepare for Director of Patient Care Services and Office
Manager review. - Communicates with Office Manager and HR to determine staffing needs and coordinates
the requests to ensure coverage in all territories. - Performs all office functions in compliance with federal, state, local laws and all policies,
procedures, and standards of Interim Healthcare Office. - Submits scheduling daily reports to Office Manager, Administrator, and Director of Patient
Care Services. - Communicates appropriately among the other Interim team members and patients.
- Completes documentation accurately, legibly, and timely for office processes
- Maintains knowledge of current regulations for home health.
- Follows policy and procedures as directed.
- Brings concerns forward appropriately to supervisor.
- Retains flexible schedules to service patients and team member requests and/or handles
complaints. - Audits missed visit report to ensure all patient visits are reassigned and/or captured as a
missed visit. - Promotes an environment of high morale and teamwork.
- Works with supervisor and other team members towards office and business goals.
- Takes appropriate and timely measures to meet the needs of the patients and team
members. - Maintains mature problem-solving approach under stressful circumstances.
- Follows pay and bill structures set by office. Gets appropriate approval for overtime,
rewards, or motivators for field staff, from supervisor. - Completes other assignments as requested and assigned.
- May have access to personal health information (“PHI”) necessary to fulfill the above duties
and responsibilities. Access to use and ability to disclose PHI is further defined by each
organization/department
To apply, you must be an expert on the following requirements:
- One year experience working in Home Health as a scheduler/or staffinfg experience, preferred
- Two-year Degree or equivalent years in training or work experience.
- Excellent communication skills.
- Demonstrated ability to manage multiple tasks with changing priorities.
- Proficient in Windows, Word, and Excel.
- Proven ability to work independently and within a team environment.
The successful candidate must submit the following pre-employment requirements:
- Scanned copy of valid NBI Clearance
- Accomplished Medical or PEME Slip (covered by KMC)
- 2x2 & Half body picture with white background
- Proof of government numbers (TIN, SSS, Pag-ibig, & Philhealth).
- Photocopy of 2 valid IDs – front & back (government-issued)
- Clear copy of your Birth Certificate (PSA or NSO)
- Accomplished HR Forms & Promissory Note (will be provided by KMC’s Onboarding Team)
