The facility, known for its exceptional long-term care rating of High Performing, provides both short-term rehabilitation and long-term care services. With 159 beds, our large facility is committed to delivering superior rehabilitation services to our residents.

Facility Details:

• Total Beds: 159
• Short-Term Rehabilitation Rating: Average
• Long-Term Care Rating: High Performing

Responsibilities:

• Provide comprehensive physical therapy services to residents.
• Develop and implement individualized therapy plans.
• Collaborate with other healthcare professionals to ensure quality care.
• Participate in interdisciplinary team meetings to discuss and coordinate resident care.
• Document resident progress and modify treatment plans as necessary.
• Educate residents and their families on continued care and exercises to be done at home.

Qualifications:

• Licensed Physical Therapist.
• Strong interpersonal and communication skills.
• Ability to work effectively within a team environment.
• Commitment to providing high-quality care and services to residents.
