About the Role:

We are seeking a licensed Occupational Therapist to provide in-home Occupational Therapy services to patients in Springfield, IL, under the direction of the attending physician. This role involves conducting assessments, developing treatment plans, and supporting patients and their families with therapeutic, self-care procedures. Your Mission:

• Assess & Plan: Conduct Occupational Therapy assessments to establish patient goals, improving physical and cognitive functioning.
• Implement Therapy Programs: Develop and execute therapy programs, as approved by the physician, to meet short and long-term patient goals.
• Patient & Family Instruction: Provide guidance to patients and family members on therapeutic procedures, adaptive techniques, and assistive equipment use.
• Progress Monitoring: Conduct regular evaluations and maintain accurate clinical notes on patient progress, reporting to physicians as needed.
• Collaborate with the Healthcare Team: Offer consultations to other team members, attend in-service meetings, and participate in staff development.

Core Competencies:

• Strong understanding of occupational medicine and social work principles.
• Ability to design and customize assistive devices for patient use.
• Proficiency in recording patient progress and maintaining clear communication with healthcare providers.
• Commitment to professional development through continuous learning.

What You Bring:

• Education: Bachelor's degree or higher in Occupational Therapy.
• Licensure: Valid Occupational Therapist, Registered (OTR) license in Illinois (IL).
• Certifications: Valid AHA Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS), or Pediatric Advanced Life Support (PALS).
• Experience: Minimum of 1 year in Occupational Medicine and 2 years in Social Work.
• Driver's License: Valid regular driver's license.

Schedule & Shift Details:

• Flexible Schedule: Weekday schedule with some flexibility based on patient needs; no on-call unless specified.
• Travel: Local travel within Springfield, IL, and surrounding areas.

Compensation & Benefits:

• Compensation Structure: Pay-Per-Visit (PPV) rates based on service and experience level.
• Bonus: $10,000 First Year Success Bonus.
