About the Role:

We're looking for a compassionate and skilled Registered Nurse (RN) to join our clients home health team. This role involves delivering patient care in home settings across Carnegie, Crafton, PA, and nearby areas. The ideal candidate will be responsible for creating, managing, and implementing care plans, providing high-quality clinical documentation, and ensuring critical deadlines are met.

Your Mission:

• Patient Care Management: Lead the delivery of patient care plans and collaborate with the healthcare team.
• Documentation: Meet critical charting deadlines to ensure accurate clinical records.
• Travel & Coordination: Manage daily schedules with local travel to patient homes (approximately 20% travel time).
• Self-Management: Demonstrate strong organizational skills to self-manage schedule and patient needs effectively.

Key Responsibilities:

• Schedule: On-call Friday, Saturday, and Sunday, 5:00 PM - 10:00 PM, with 8 units on Saturday and Sunday; 2 holidays per year.
• Travel Requirements: Local travel to patient locations within the Carnegie/Crafton area.

Requirements:

• Education: Associate degree or higher in Nursing.
• Experience: Minimum of 1 year in clinical documentation, informatics, or quality, plus 1 year in EMR systems (EMR, Practice Partner).
• Licensure: Valid Registered Nurse (RN) license in Pennsylvania (PA).
• Certifications: Advanced Life Support (ARC ALS) and Advanced Cardiac Life Support (AHA ACLS) certifications.
• Driver's License: Valid driver's license.

Compensation & Benefits:

• Compensation Structure: Pay-Per-Visit rates ($42-46 per visit) with a guaranteed $3K stipend per pay period during the 6-month orientation.
• Sign-On Bonus: $15,000 (paid quarterly over one year).
• Performance Bonuses: Outcome-based bonuses of $1,000 per quarter.
• Estimated Earnings: $77,000 - $100,000+ annually.
