information or documentation indicated in such form, which also describes the procedure to

resolve your request.

Data Subject’s Rights and Revocation of Consent

You will have the right, either directly or through a legal representative, to access, rectify, cancel

your personal data, according to the terms and subject to the exceptions established in the Law,
