What You Bring to the Table:

  • BSN or higher in Nursing (required)

  • Active RN license in Arizona or Compact State (required)

  • BLS certification upon hire

  • 2+ years as a Case Management Director in a critical care hospital (required)

Preferred:

  • CCM or ACM certification

  • Master's degree

  • 5+ years leadership in case management or critical care hospital management

  • Enhance care coordination, patient advocacy, and discharge planning

  • Collaborate across departments to integrate social work, nursing, and physician input into care plans

  • Lead and evaluate performance improvement initiatives to maximize quality outcomes and cost-effectiveness

  • Manage and support your team's growth through leadership, education, and mentorship

  • Conduct concurrent medical record reviews, address adverse events, and manage utilization resources

  • Facilitate interdisciplinary rounds, promote family education, and lead community-focused initiatives
