This full-time opportunity is ideal for a compassionate clinician who is passionate about helping older adults maintain mobility, independence, and a high quality of life through personalized, patient-centered rehabilitation services.
Unlike traditional high-volume therapy environments, this position emphasizes meaningful patient interactions, coordinated interdisciplinary care, and manageable caseloads that allow therapists to focus on long-term outcomes instead of rushed appointments. The successful candidate will provide evidence-based rehabilitation services while collaborating with healthcare professionals dedicated to supporting seniors within their communities.
Key Responsibilities
- Perform comprehensive physical therapy evaluations and develop individualized treatment plans.
- Deliver evidence-based therapeutic interventions focused on improving mobility, balance, strength, endurance, and functional independence.
- Monitor patient progress and modify treatment plans based on clinical outcomes and rehabilitation goals.
- Collaborate with physicians, nurses, occupational therapists, social workers, and interdisciplinary care teams.
- Educate patients and caregivers on injury prevention, therapeutic exercises, mobility strategies, and home exercise programs.
- Maintain accurate and timely clinical documentation while meeting regulatory and organizational standards.
- Participate in interdisciplinary case conferences and care planning meetings.
- Promote patient safety while supporting rehabilitation best practices.
- Build positive relationships with patients, families, and healthcare team members.
- Contribute to quality improvement initiatives that enhance patient outcomes and clinical excellence.
Required Qualifications
- Current Physical Therapist (PT) license in California or eligibility to obtain California licensure before employment.
- Doctor of Physical Therapy (DPT) or equivalent accredited Physical Therapy degree.
- Strong clinical assessment and treatment planning abilities.
- Experience treating adult or geriatric patient populations.
- Excellent communication, patient education, and interpersonal skills.
- Ability to work effectively within an interdisciplinary healthcare team.
- Strong organizational skills and commitment to high-quality patient care.
- Must currently reside within commuting distance of the San Francisco Bay Area or be prepared to relocate independently before employment.
- Authorized to work in the United States.

Preferred Qualifications
- Experience working with medically complex geriatric patients.
- Background in outpatient rehabilitation, home health, community-based rehabilitation, or post-acute care.
- Experience with fall prevention, balance training, neurological rehabilitation, and functional mobility programs.
- Board Certification or advanced rehabilitation training.
- Knowledge of Medicare documentation and rehabilitation compliance requirements.
- Experience working within interdisciplinary care models.
- Stable employment history demonstrating progressive clinical growth and patient care excellence.
HireNow Package
- Compensation: $115,000–$136,000 annually
- Performance Bonus: Bonus opportunity available.
