HRRMC South Park Health Center is seeking a Full-Time Licensed Practical Nurse. This position is scheduled for 40 hours/week Monday-Friday. 

SUMMARY STATEMENT:

The Licensed Practical Nurse (LPN) provides care in the clinic setting at the direction of the Registered Nurse (RN). The LPN aid patients from the moment they enter the clinic. The LPN contributes to the patient assessment of health status through the collection, reporting and
documenting of objective and subjective data, observes patient condition for changes and reports signs and symptoms of deviation from normal health status to the RN and/or physician. This role plans, directs, and performs quality nursing care for all clinic patients as well as provides
high quality customer service and patient care while facilitating real-time resolution to any customer concerns with support of the department manager.

ESSENTIAL DUTIES PER AREA OF RESPONSIBILITY

a. Models positivity with all interactions, internally and externally, presenting the organization in a positive manner.
b. Communicates patient concerns or issues directly with their provider whenever possible and consults immediately with their immediate supervisor if there is not a satisfactory resolution as well as non-patient related issues/challenges. Does not wait to be rounded on to share concerns with their immediate supervisor.
c. Quickly masters and then maintains mastery level skill set with regards to the clinic/organization’s electronic medical records systems; is proficient at utilizing the computer and internet; manages telephone calls, performs patient assessments, documents in the electronic health record, and assists patients and providers as needed in the practice setting.
d. Communicates any patient concerns or issues directly with their provider
whenever possible and consults immediately with their immediate supervisor if there is not a satisfactory resolution, and if there are any non-patient concerns or issues. Does not wait to be rounded on to share concerns.
e. Participates in daily practice huddles reviewing staffing, tracking reports (orders sent and results received), patient phone calls, as well as reviewing the provider/patient schedule for the day, identifying any challenges (patient or scheduling) and collaborating with the clinic team on developing and implementing strategies to manage these challenges; seeks supervision as needed.
f. Ensures that patient phone calls are returned daily and consistent with HRRMC Communication Standards Policy.
g. When assisting in performing a patient procedure, ensures that a time out takes place before proceeding; seeks supervision if the performing provider resists.
h. Monitors to ensure that their provider is checking medical necessity for any/all orders entered, enlisting the assistance of a supervisor when needed.
i. Understands requirements for any quality payment incentive programs and takes ownership where indicated to ensure the nursing component is completed 100% of the time. Monitors their provider and alerts a supervisor regarding any challenges associated with the provider completing their part.
j. Acts as a liaison between the hospital and local/consulting physician offices to thanking them for any referrals as well as closing the patient referral loop; fosters positive/cooperative relationships and problem solves issues regarding the clinic.
k. Cheerfully assists other nursing units/clinics during shortage and/or whenever requested.
l. Assists in arranging for referrals to other medical or community agencies those patients who need further evaluation or treatment such as Home Health and Hospice, home oxygen, and physical therapy.
m. Assists providers and patients with scheduling patients for ancillary services such as imaging, lab, infusion, cardiology procedures, etc. Communicates challenges promptly to their immediate supervisor.
n. As assigned by their immediate supervisor, may develop clinical guidelines in collaboration medical staff, clinic leadership, pharmacy, and other ancillary services as appropriate.
o. Maintains knowledge and skill sets required to conduct patient education session related to specific clinic area.
p. Provides patient care as assigned by the Registered Nurse or the physician.
q. Acts as a liaison between the hospital and local/consulting physician offices to foster cooperative relationships and problem solves issues regarding the clinic.
r. Performs other duties as request and within the scope of practice for their position.

EDUCATION and/or EXPERIENCE

Graduate from an approved vocational licensed practical nurse program

CERTIFICATES, LICENSES, REGISTRATIONS

Must maintain current, active license as a Licensed Practical Nurse in the State of Colorado. Must maintain current BLS certification.
