Located in Toronto, Ontario, this full-time, permanent position offers a salary of $63,071 to $73,447, with two openings available in a 24/7 trauma-informed residential program supporting female youth aged 12 to 17.
About the Opportunity
HealthOPM is recruiting for a Toronto-based non-profit organization offering specialized anti-human trafficking live-in programs for vulnerable youth.
As a Clinical Care Coordinator, you'll conduct assessments, facilitate evidence-informed therapy, and ensure treatment fidelity while leading trauma recovery initiatives.
Key Responsibilities

Lead client intake, assessment, and eligibility screening

Provide trauma-focused therapy and group facilitation

Coordinate individualized treatment and complex-care plans

Maintain detailed records in TREAT and ensure compliance

Navigate internal/external services to ensure continuity of care
Qualifications & Requirements

Bachelors degree (Masters preferred) in Social Work or Psychology

Registered with OCSWSSW or CRPO

Minimum 3 years experience delivering clinical services to exploited youth

Proficiency in trauma-informed, harm-reduction, and anti-oppressive practices
