We are looking for Registered Practical Nurses (RPN) in a Long-Term Care Setting in Hawkesbury.
Open to Full - time hours, part - time hours or casual!
Shifts : 8 - hour shifts ( 7:00am - 3:00pm / 3:00pm -11:00pm / 11:00pm -7:00am)

Key Responsibilities:
- Patient Care & Assessment:
- Conduct thorough assessments of residents' health conditions and medical histories.
- Develop, implement, and update individualized care plans based on resident needs.
- Monitor and document vital signs, symptoms, and changes in patient conditions.
- Provide hands-on care, including administering medications, wound care, and other nursing interventions.
- Collaborate with the interdisciplinary care team (physicians, social workers, therapists) to ensure comprehensive care.
- Medication Management:
- Administer prescribed medications and treatments as per healthcare provider orders.
- Monitor for adverse effects and report any complications to the healthcare team.
- Educate residents and families on proper medication administration and side effects.
- Resident Advocacy:
- Advocate for the physical, emotional, and social well-being of residents.
- Provide guidance and support to family members regarding the care of their loved ones.
- Assist in coordinating activities that promote the overall quality of life for residents.
- Leadership & Collaboration:
- Supervise and mentor nursing assistants, LPNs, and other clinical staff.
- Ensure compliance with facility policies, state, and federal regulations.
- Participate in staff meetings, care plan meetings, and other collaborative efforts to improve care delivery.
- Documentation & Compliance:
- Maintain accurate and timely medical records, ensuring compliance with HIPAA and other healthcare regulations.
- Complete incident reports and other documentation as necessary.
- Ensure care delivery meets or exceeds facility and regulatory standards.
Qualifications:
- Education:
- Bachelor of Science in Nursing (BSN)
- Current Registered Nurse and Registered Practical Nurse License from CNO
- CPR and First Aid certification (or ability to obtain upon hire).
- Experience:
- Previous experience in long-term care, geriatrics, or a similar healthcare setting preferred.
- Strong clinical assessment skills and the ability to recognize changes in resident health status.
- Excellent communication and interpersonal skills to work effectively with residents, families, and the healthcare team.
- Skills & Abilities:
- Knowledge of nursing principles, practices, and regulations.
- Ability to manage a diverse caseload and prioritize tasks in a fast-paced environment.
- Strong organizational and problem-solving skills.
- Compassionate, patient- centered approach to nursing care.
