Essential Responsibilities:

• Conduct comprehensive patient evaluations and develop individualized treatment plans
• Deliver evidence-based therapeutic interventions across diverse patient populations
• Document patient progress and maintain detailed clinical records
• Collaborate with interdisciplinary healthcare team members
• Participate in care planning meetings and clinical documentation
• Mentor and supervise physical therapy aides
• Maintain compliance with all regulatory requirements

Required Qualifications:

• Graduate degree from CAPTE-accredited Physical Therapy program
• Current New York State Physical Therapy license
• Strong clinical assessment and treatment skills
• Excellent communication and interpersonal abilities
• EMR proficiency

Preferred Qualifications:

• 2+ years of clinical experience
• Experience in geriatric and subacute care
• Leadership or supervisory experience
