🩺 Position Overview

As a Physical Therapist, youll provide evidence-based therapeutic care across multiple settings including outpatient rehab, skilled nursing, and assisted living facilities. Youll be empowered to design customized treatment plans, collaborate with interdisciplinary teams, and make a meaningful impact on the lives of adult and geriatric patients. Key Responsibilities

• Develop and implement individualized treatment plans based on physician referrals

• Conduct patient evaluations and deliver therapeutic interventions

• Track progress through detailed documentation and regular assessments

• Collaborate with nurses, therapists, and support staff to optimize outcomes

• Participate in care planning and team meetings

• Provide education and support to patients, families, and staff

• Offer leadership support in the Directors absence

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or equivalent)

• Active New York State PT license (or eligibility)

• Strong interest in adult, subacute, and geriatric care

• Excellent communication and team collaboration skills

• Leadership ability and willingness to mentor rehab aides or junior staff

• Experience preferred, but new graduates are encouraged to apply!
