Senior MDS Assessment Coordinator - Long Term Care (RN)

A leading Long-Term Care and Rehabilitation Facility in the Bronx, NY seeks an experienced MDS Assessment Coordinator to join our dedicated healthcare team. This role offers an excellent opportunity to utilize your clinical expertise and leadership skills in a supportive environment while making a meaningful impact on resident care.

Essential Responsibilities:

  • Lead and manage the complete MDS assessment process, ensuring accuracy and compliance with federal guidelines
  • Coordinate with the Interdisciplinary Team to develop comprehensive care plans
  • Monitor and optimize RUG scores for appropriate reimbursement under PDPM
  • Conduct quarterly reviews and updates of resident assessments
  • Oversee care plan scheduling and implementation
  • Ensure timely completion and submission of all MDS documentation
  • Provide guidance to staff regarding resident care changes and documentation requirements
  • Maintain strict confidentiality of resident and organizational information

Required Qualifications:

  • Active New York State Registered Nurse (RN) License
  • Minimum 2 years Long Term Care experience
  • Demonstrated expertise in PDPM and CMI methodologies
  • Prior MDS 3.0 assessment experience
  • Strong organizational and leadership abilities
  • Excellent communication and interpersonal skills
