Job Description

Community Engagement & Development

  • Build trust-based relationships with residents and local stakeholders in Tengah New Town and surrounding precincts to identify community strengths, informal support networks, and shared priorities.
  • Conduct placed-based outreach, including 1:1 conversation, to understand individual and community aspiration, using both lived experience and data to inform engagement strategies.
  • Facilitate community-led efforts that promote social connection, mutual support and neighbourhood level wellbeing.
  • Support residents in navigating health and social systems, with an emphasis on enabling independence and long-term resilience.

Partnership Development

  • Maintain active partnerships with internal NUHS teams and external health, social service, and community agencies to coordinate care and co-create community-based solutions.
  • Facilitate information-sharing and case discussions between hospital and community partners to close care gaps.

Capability Building

  • Promote strengths-based, community-first approaches among clinical and frontline teams through coaching and sharing of practice tools.
  • Contribute to training efforts that embed asset-based thinking into transitional care planning and service design.
  • Support development of evaluation metrics that reflect both individual outcomes and community contributions to care.

Transitional Care Integration

  • Work with clinical teams to develop community-anchored discharge plans, particularly for patients with complex needs or limited support systems.
  • Connect residents to local resources that reduce hospital readmissions and support recovery at home or in the community.
  • Monitor outcomes and contribute to process improvements that strengthen post-discharge continuity of care.

Administrative

  • Prepare periodic reports to update the Hospital Committee.
  • Any other projects/tasks assigned by the Reporting Officer.

Job Requirements

  • Bachelors degree in community development, public health, social work or related field.
  • 3-5 years of work experience in population health, care integration or community development settings.
  • Familiarity with strengths-based approaches and participatory methods.
  • Competence in programme evaluation and basic data analysis.
  • Ability to work across sectors and navigate complexity in health and social care settings.
  • Strong communication, facilitation and relationship-building skills.
