About the Role:

The client is seeking a dedicated Overnight Telehealth Nurse Practitioner (ONTH) to join their fully remote care team. This role involves providing high-quality virtual care to geriatric patients in SNFs across multiple states. The ideal candidate is experienced, compassionate, and comfortable managing complex cases independently during nights, weekends, and holidays, with virtual support and collaboration from a multidisciplinary team.

Key Responsibilities:

• Provide remote telehealth evaluations and management for geriatric patients in SNF settings.
• Respond promptly to urgent calls, including assessments, medication orders, and care planning.
• Collaborate with nursing staff and facility teams to support continuity of care and reduce hospital transfers.
• Document all patient interactions, assessments, and recommendations in the EMR system in a timely manner.
• Participate in ongoing clinical communication with physicians and other NPs as needed.
• Maintain availability for a mix of overnight and weekend shifts (including holidays).

Qualifications:

• Active CNP licenses in at least 2 of the following states: MA, NJ, NH, CT, NY, VT, FL, TX is required. (nice to have at least 4+ active licenses in the listed states).
• Board Certified.
• Minimum 2 years as a practicing Nurse Practitioner.
• Experience with geriatric populations required.
• Comfortable with telehealth platforms and electronic medical records.
• Willing to work a flexible schedule that includes overnights, weekends, and holidays.
