Clinical Nurse Coordinator – Remote
Location: Remote – South Africa
Working Hours: 3:00 PM- 11:00 PM, Monday–Friday (SA Time)

About the Role
We are seeking a clinically oriented Nurse Coordinator to support patient services and ensure that patients receive appropriate clinical coordination and documentation. The ideal candidate will have nursing or relevant clinical experience and be comfortable communicating with the Director and Patient Services team regarding clinical needs.
Key Responsibilities
- Coordinate and schedule biannual nursing assessments for patients.
- Communicate with the Director and Patient Services team regarding clinical needs and patient concerns.
- Prepare and send required clinical documentation to patients' physicians, including documents via fax.
- Inform physicians of the patient's approved hours and services being provided, as required for Medicaid compliance.
- Prepare and submit required reports to the state when a patient is admitted to or visits a hospital.
- Complete and maintain incident reports accurately and promptly.
- Ensure clinical records and documentation are complete, accurate, and up to date.
- Support compliance with state and Medicaid requirements.
- Liaise professionally with physicians, patients, families, and internal clinical teams.
- Escalate clinical concerns appropriately to the Director or relevant clinical personnel.
NHTD Programme
Experience with NHTD (Nursing Home Transition and Diversion) is beneficial but training will be provided.
The programme supports individuals who require assistance to remain safely in the community, including patients with dementia and other complex care needs.
Requirements
- Nursing qualification or relevant clinical background preferred.
- Previous clinical experience strongly preferred.
- Strong understanding of clinical documentation and patient care processes.
- Excellent written and verbal English communication skills.
- Ability to communicate professionally with physicians and clinical teams.
- Strong attention to detail and accuracy.
- Comfortable handling confidential patient information.
- Good organisational and administrative skills.
- Ability to work independently in a remote environment.
- NHTD experience is an advantage, but training will be provided.

- Ability to work consistently during 9:00 AM – 5:00 PM SA Time, Monday–Friday.
